Report Fraud      

You can report suspected fraud, waste, or abuse in Texas health and human services programs by filling out this form.  The Texas health and human services system includes:

  • Medicaid  
  • SNAP, the Supplemental Nutrition Assistance Program or Food Stamps  
  • TANF, Temporary Assistance for Needy Families  
  • WIC, Women, Infants, and Children program  
  • Texas State Supported Living Centers  
  • CHIP, the Children's Health Insurance Program  

Please provide as much information as possible.  Things that are important to provide include:

  • The date of birth and Social Security number of the person you suspect of committing fraud  
  • The phone number and address of the person you suspect  
  • Other information you think might be helpful to an investigator  

Your information remains confidential.

We will only share your identity and contact information with our investigators.  OIG is prohibited by law from releasing your name, or any other information regarding your fraud referral.

OIG is prohibited from providing you with an update on the status of any open investigation.

At the conclusion of your referral, you will receive a tracking number.  Keep this number handy, as it will be necessary for you to provide if you have more information to add at a later time.
